Frontline staff and management assessment of recent QAPI work
(Project Evaluator will complete report)

Qualidigm will facilitate a one-day session with the QAPI Committees from both facilities to share and assess results to date and receive additional training

Assessments:
Frontline staff and management assessment of recent QAPI work

QAPI Committee Meetings and Coaching:
QAPI Committees will meet twice each month for two hours, for three months, then once each month for the duration of the project
Qualidigm will provide coaching at each committee meeting to facilitate appropriate methodologies are used to insure success
Training furnd and Union staff will attend all committee meetings to offer insight, encouragement, and logistical support as needed

Assessments - Conduct baseline surveys including:

Frontline staff and Management assessment of overall employee satisfaction
Frontline staff and management assessment of current L-M relations

QAPI Committee Meetings and Coaching:
Establish committee meeting schedule in each facility, and insure that there will be coverage for frontline staff during committee meetings

Kick-Off Training:
Plan and finalize training content with FMCS and Qualidigm
Decide on date and schedule coverage for frontline workers
Conduct training with full participation from both facilities QAPI Committees, Training Fund staff and Union organizers

Establish QAPI Committee in each facility:
Facility Management and Union Leaders with input from Training Fund and Qualidigm will select appropriate committee members
Committee charter will be signed by all committee members defining roles and commitments
